I dunno - he seems to be going up and to the right, suggesting he's coming from the lava. Also he doesn't appear to be on the same plane as the background.
I dunno dunno though yanow - there's never an instance in the game where there's bricks in the background plane, and the motion lines behind him suggest that he's about to ram into the wall. Also, there's no possible way he could jump UP from the lava, considering any contact at all would kill the jumpy lil' bastard. That all being said, he's shooting a fireball through a brick wall while he's touching lava, so I'm doubting that the cover art follows game logic at all.
