Getting CJ with over 50% fat unlocks alternate dialogue. For example, if CJ gets obese before Sweet's phone call to go to the gym, Sweet will take note on how CJ is "letting himself go".

If you fail the mission Running Dog while being fat, Big Smoke will call you to go to the gym as well (him of all people).

Any missions involving the jetpack, such as Black Project and Green Goo, will not start if Carl is too obese, because it would take a lot more power for him to be propelled.

I didn't see the use of getting Fit CJ or Fat CJ so never noticed

Yeah, it's real easy to get him fat, but it takes a lot of time to get him fit again, so I never bothered getting him fat. The only thing I had to worry was making sure his fat never reached zero or he'd starve and lose health.
