The debugging version of OoT contains a ton of data not used in the final version of the game. Among them is a neck-less test render of a character who displays improperly and creepily has mouths where its eyes should be.

Apparently Nintendo was in on this "mouth eyes" fad long before the Internet caught wind of it.

Quote:It's quite possible that this actor was simply a test for the animations for other actors in the game, as it has most, if not all, of the animations for the other actors. She also appears to have another mouth a little below her nose if you look closely. This would seem to indicate that the "lip" texture is repeated over the entire model. The model is an earlier version of the Kakariko housewife character, and parts of it aren't fully connected. Unlike the other characters, the actor's name is simply "object_bob", which may be some form of developer in-joke.

This is the second DYK that I've sourced to The Cutting Room Floor Wiki. There's a ton of solid, awesome content here. I highly suggest checking it out for future DYK's. I won't post any more sourced from here, though, just because I don't want to stagnate the forum with a ton of content from one site.
Jesus christ, how horrifying.

That's some Shadow Temple/Well level stuff right there.

Where we're going we won't need eyes to see.

Here's two extra mouths.
I don't know what's worse - the image itself, or the fact that I now know that somewhere, deep down in my OoT cartridge, this abonimation is lurking, always remaining beyond the shadows
